Tribute Gift
Here's your opportunity to make a donation in honor of a friend or relative
or someone important to you. When you make a tribute gift, we ask
for your name, address and the honoree's name and address.
We'll send a card to the honoree acknowledging your thoughtfulness
(you can add your own message too). You'll also receive a card
from HDSA to confirm your donation and advising you that a gift card
has been sent to the honoree. All Tribute gifts will be acknowledged
(with your permission) in the HDSA Newsletter, The Marker.

Memorial Gift
A meaningful way to honor the memory of a loved one.
We ask you to fill in your name and address, the deceased person's name,
and the name and address of the person to whom you want us to send
an acknowledgment (to you or to the person you designate as the recipient).
You can add your own message to the card. You'll receive a card
from HDSA confirming your donation and advising you a gift card
has been sent in your name. All Memorial Gifts will be acknowledged
(with your permission) in the HDSA Newsletter.
